Background:

  • Erythrokeratodermas encompass inherited or sporadic disorders of keratinization.
  • Progressive symmetrical erythrokeratoderma is characterized by fixed, symmetrical plaques with erythema or hyperpigmentation and hyperkeratosis.

Observation:

  • A 9-year-old boy presented with progressive symmetrical erythrokeratoderma.
  • He exhibited typical fixed hyperpigmented, keratotic plaques along with rare perioral keratotic lesions.

Findings:

  • The patient showed an unusual presentation of progressive symmetrical erythrokeratoderma with perioral involvement.
  • Initial treatment with oral etretinate yielded no response.
  • Topical keratolytics provided minimal improvement.

Implications:

  • This case expands the known clinical spectrum of progressive symmetrical erythrokeratoderma.
  • It underscores the challenges in managing this rare genodermatosis.
  • Further research into effective therapeutic strategies for erythrokeratodermas is warranted.
