Summary

Primary diffuse leptomeningeal oligodendrogliomatosis, a rare brain and spine cancer, was diagnosed postmortem in a 25-year-old woman. This case highlights its potential for sudden death, even without increased intracranial pressure.

Background:

  • Primary diffuse leptomeningeal oligodendrogliomatosis is an exceptionally rare central nervous system malignancy.
  • It is characterized by diffuse leptomeningeal infiltration without an identifiable primary intraparenchymal tumor focus.

Observation:

  • A 25-year-old woman presented with a 15-month history of paraplegia and sensory loss (thoracic 8-9 level).
  • She experienced no symptoms indicative of increased intracranial pressure.
  • The patient died unexpectedly before a planned biopsy.

Findings:

  • Postmortem examination revealed a nodular lesion at the thoracic 10 vertebra.
  • Diffuse oligodendroglioma infiltration was found throughout the leptomeninges of the brain and spine.
  • No intra-axial tumor focus was identified.

Implications:

  • This case represents the first documented instance of primary diffuse leptomeningeal oligodendrogliomatosis leading to sudden death.
  • It underscores the aggressive potential and diagnostic challenges of this rare condition.
  • Highlights the importance of considering leptomeningeal disease in unexplained neurological deficits.
