Background:

  • Catamenial pneumothorax is a rare condition characterized by recurrent spontaneous pneumothorax occurring in relation to the menstrual cycle.
  • The exact etiology remains unclear, but diaphragmatic defects and endometriosis are implicated.

Observation:

  • Three cases of catamenial pneumothorax are presented, involving recurrent right-sided pneumothorax in women aged 38-40.
  • Diagnostic videothoracoscopy revealed diaphragmatic fenestrations and, in one case, visceral pleural endometriosis.
  • Surgical interventions included diaphragmatic resection or plication and bulla resection.

Findings:

  • Two patients initially treated with diaphragmatic resection and hormone therapy experienced recurrence.
  • One patient with diaphragmatic fenestrations and bullae, treated with resection and hormone therapy, had a recurrence after 4 years, successfully managed with diaphragm reefing and bulla resection.
  • Another patient with diaphragmatic fenestrations underwent resection and hormone therapy, but recurred and was found to have visceral pleural endometriosis, treated again with hormone therapy.

Implications:

  • Combined surgical and hormonal therapy appears beneficial for managing catamenial pneumothorax.
  • Recurrence suggests the need for tailored treatment strategies, potentially involving repeat surgery or extended hormonal management.
  • Further research is needed to elucidate the pathophysiology and optimize treatment protocols for this rare condition.
