Background:

Community clinics often fail to provide adequate breast cancer screening for low-income, diverse populations.
Existing breast cancer screening delivery models require enhancement to improve patient outcomes.

Purpose of the Study:

To develop and evaluate multifaceted organizational system interventions aimed at increasing mammography rescreening rates in community clinics.
To assess the impact of redesigning breast screening delivery services using the Chronic Care Model and Put Prevention Into Practice framework.

Main Methods:

A quasi-experimental design was employed with random sampling of patients pre- and post-intervention.
Interventions included operational assessments, tracking systems, reminder calls, tailored education, physician prompts, and counseling calls.
Chart abstraction was used to determine rescreening rates in a sample of low-income women aged 50+ with normal mammography results.

Main Results:

Organizational system redesign significantly improved mammography rescreening rates from 32.1% to 50.2% (p < .001).
The addition of a tailored counseling call to the redesigned system showed maintenance of rescreening rates (44.4% to 45.1%, p > 0.05).

Conclusions:

Multilevel interventions targeting community clinic primary care services can effectively enhance mammography rescreening rates.
Redesigning clinic systems is a viable strategy to improve breast cancer screening adherence in vulnerable populations.
