Summary

Granular cell astrocytoma (GCA) is a rare, aggressive brain tumor. This case study highlights its unique ultrastructural features, suggesting mitochondria and paracrystalline inclusions contribute to its granular appearance.

  • Granular cell astrocytoma (GCA) is a rare astrocytoma variant with a poor prognosis.
  • Distinguishing GCA from non-neoplastic conditions like infarcts or demyelinating diseases can be challenging due to similar cytologic and histologic features.
  • This study details a GCA case in a pregnant woman presenting with seizures during delivery.

Observation:

  • Histopathological and immunohistochemical findings were consistent with previous GCA reports.
  • Ultrastructural analysis revealed fewer secondary lysosome granules than expected.
  • Numerous mitochondria and unique paracrystalline inclusions were observed within tumor cells.

Findings:

  • The granular appearance in light microscopy may result from both phagolysosomes and abundant mitochondria/paracrystalline inclusions.
