Vertebral artery hypoplasia: a predisposing factor for posterior circulation stroke?

Vertebral artery hypoplasia (VAH) is significantly more common in patients experiencing posterior circulation strokes. This finding suggests VAH may predispose individuals to this type of stroke.

- Color-coded duplex flow imaging was used to assess the V2 segment of the vertebral arteries.
- Vertebral artery hypoplasia was defined as a diameter less than or equal to 2.5 mm.

- Vertebral artery hypoplasia (VAH) was identified in 13% of patients with posterior circulation infarction.
- In contrast, VAH was found in only 4.6% of patients with strokes in other territories.
- This difference in VAH prevalence between stroke territories was statistically significant (p < 0.001).
- Common stroke risk factors (hypertension, hyperlipidemia, diabetes, smoking) showed no significant difference in distribution between the groups.
Conclusions:
- Patients with vertebral artery hypoplasia (VAH) appear to have a predisposition to stroke within the posterior circulation territory.
- VAH may be an independent risk factor for posterior circulation stroke.
- Further research is warranted to elucidate the mechanisms underlying this association.
