Summary

A sphenoid sinus mucocele caused sudden vision loss, mimicking optic neuritis. Surgical drainage of the mucocele significantly restored the patient's vision.

Background:

  • Optic neuritis is a common cause of acute visual loss.
  • Sphenoid sinus mucoceles are rare benign cystic masses that can cause significant complications due to their proximity to critical neurovascular structures.

Observation:

  • A 36-year-old man presented with acute left eye visual loss.
  • Cranial MRI revealed a sphenoid sinus mucocele compressing the left optic nerve via extension into the anterior clinoid process.

Findings:

  • Endoscopic marsupialization of the sphenoid sinus mucocele was performed.
  • The patient experienced marked improvement in vision following the surgical intervention.

Implications:

  • Sphenoid sinus mucocele should be considered in the differential diagnosis of optic neuritis.
  • Endoscopic surgery is an effective treatment for vision-threatening sphenoid sinus mucoceles.
