Background:

Peripheral odontogenic fibroma is a rare gingival lesion.
It can clinically mimic other oral pathologies, including reactive lesions, benign neoplasms, and metastases.
Accurate diagnosis is crucial for appropriate management.

Observation:

A symptomatic lesion on the mandibular gingiva of a 58-year-old female was analyzed.
The patient had no history of trauma or prior dental disease in the affected area.
Clinical presentation mimicked various other oral conditions.

Findings:

Excisional biopsy revealed a peripheral granular cell odontogenic fibroma.
The stroma contained numerous polyhedral granular cells.
Interspersed islands of inactive odontogenic epithelium were noted.

Implications:

This represents the fourth detailed case report of peripheral granular cell odontogenic fibroma in the literature.
The findings contribute to understanding the histopathological spectrum of rare odontogenic tumors.
Increased awareness may improve diagnostic accuracy for similar challenging gingival lesions.
