Background:

  • Peripheral odontogenic fibroma is a rare gingival lesion.
  • It can clinically mimic other oral pathologies, including reactive lesions, benign neoplasms, and metastases.
  • Accurate diagnosis is crucial for appropriate management.

Observation:

  • A symptomatic lesion on the mandibular gingiva of a 58-year-old female was analyzed.
  • The patient had no history of trauma or prior dental disease in the affected area.
  • Clinical presentation mimicked various other oral conditions.

Findings:

  • Excisional biopsy revealed a peripheral granular cell odontogenic fibroma.
  • The stroma contained numerous polyhedral granular cells.
  • Interspersed islands of inactive odontogenic epithelium were noted.

Implications:

  • This represents the fourth detailed case report of peripheral granular cell odontogenic fibroma in the literature.
  • The findings contribute to understanding the histopathological spectrum of rare odontogenic tumors.
  • Increased awareness may improve diagnostic accuracy for similar challenging gingival lesions.
