Hereditary multiple exostoses (HME) can cause rare popliteal artery pseudoaneurysms in young patients. Early diagnosis via advanced imaging and surgical repair with a saphenous graft are crucial for managing this vascular complication.

Background:

  • Hereditary multiple exostoses (HME) is a genetic disorder characterized by the development of multiple osteochondromas.
  • Vascular complications, though rare, can occur in HME patients, with pseudoaneurysms being the most frequent.
  • The popliteal artery is a common site for pseudoaneurysm formation in the context of HME.

Observation:

  • A 16-year-old male with HME presented with a pseudoaneurysm of the left popliteal artery.
  • The pseudoaneurysm was attributed to an osteochondroma originating from the lower femur.
  • Diagnosis was established using ultrasound, MRI, and MR angiography, obviating the need for conventional angiography.

Findings:

  • Surgical excision of the osteochondroma was performed.
  • The damaged popliteal artery was successfully repaired using a saphenous vein graft.
  • This case highlights the importance of considering pseudoaneurysm in HME patients presenting with knee masses.

Implications:

  • Early recognition of pseudoaneurysm is vital in young HME patients with knee masses.
  • Non-invasive imaging modalities are effective for diagnosing this vascular complication.
  • Timely surgical intervention can lead to successful outcomes and prevent further complications.
