Malignant external otitis, a severe pseudomonal infection, can be effectively treated with hyperbaric oxygen therapy. This adjuvant treatment improved outcomes in advanced and recurrent cases, showing no complications.

Background:

  • Malignant external otitis (MEO) is a severe pseudomonal infection primarily affecting the elderly.
  • Conventional antipseudomonal antibiotic therapy is often prolonged and ineffective in advanced or recurrent MEO, especially in immunocompromised individuals.

Observation:

  • Sixteen patients with MEO, including six with advanced or recurrent disease unresponsive to antibiotics, received adjuvant hyperbaric oxygen (HBO) therapy.
  • A 30-day course of HBO was added to the standard antibiotic regimen.

  • Patients remained free of infection and neurological deficits during 1-4 years of follow-up.
