Summary

Asteroid bodies, a rare cytologic finding, can aid in the early diagnosis of sarcoidosis when identified in lymph node aspirates. This aids in differentiating sarcoidosis from other granulomatous conditions.

Background:

  • Sarcoidosis is a multisystem disease with diverse clinical presentations.
  • Cutaneous lesions occur in one-third of patients and may precede systemic disease.
  • Early diagnosis of sarcoidosis can be challenging.

Observation:

  • A 37-year-old woman presented with asymptomatic subcutaneous nodules and lymphadenopathy.
  • Epitrochlear lymph node aspirate revealed noncaseating epithelioid granulomas, multinucleate giant cells, and asteroid bodies.
  • Subcutaneous nodule biopsy confirmed the findings.

Findings:

  • The lymph node aspirate demonstrated characteristic features of sarcoidosis, including asteroid bodies.
  • Elevated erythrocyte sedimentation rate and angiotensin-converting enzyme levels supported the diagnosis.
