Summary

A rare spontaneous triplet pregnancy resulted in a heterotopic pregnancy, where one ectopic pregnancy was surgically removed, allowing the remaining twin intrauterine pregnancy to continue to term. This case highlights the importance of considering heterotopic pregnancy in differential diagnoses.

  • A 30-year-old multigravida presented with right lower quadrant pain, initially diagnosed via ultrasound as an early intrauterine pregnancy.
  • Laparoscopic evaluation revealed a ruptured right ectopic pregnancy, with a normal appendix.
  • The patient underwent laparoscopic salpingostomy for the ectopic gestation.

Findings:

  • Post-operatively, pelvic ultrasounds confirmed a viable spontaneous twin intrauterine pregnancy.
