Area of Science:

  • Rheumatology
  • Infectious Diseases
  • Dermatology

Background:

  • Dermatomyositis is an idiopathic inflammatory myopathy characterized by muscle weakness and skin rash.
  • Lyme disease, a tick-borne illness, can manifest with a wide range of systemic symptoms.

Observation:

  • A 73-year-old woman presented with a 5-month history of asthenia, periorbital edema, and forearm skin infiltration.
  • Clinical presentation and biopsy findings were consistent with dermatomyositis.
  • The patient was seropositive for Lyme disease.

Findings:

  • Laboratory tests revealed elevated muscle enzymes and inflammatory markers.
  • Skin and muscle biopsies confirmed dermatomyositis.
  • Successful treatment with doxycycline was observed.

Implications:

  • Lyme disease should be considered in the differential diagnosis of dermatomyositis.
  • This case underscores the importance of considering infectious etiologies in inflammatory myopathies.
  • Prompt diagnosis and treatment of Lyme disease can prevent misdiagnosis and ensure effective management.
