Summary

A rare case of common bile duct hyperplasia was misdiagnosed as adenocarcinoma due to biopsy artifacts. Pathologists should be aware of this rare biliary tract polypoid lesion.

Observation:

  • A 45-year-old Japanese woman presented with a polypoid lesion in the common bile duct.
  • Endoscopic retrograde sphincterotomy and biopsy suggested adenocarcinoma preoperatively.

Findings:

  • Final histopathological diagnosis after pancreatoduodenectomy revealed hyperplasia, not adenocarcinoma.
  • Diagnostic discrepancy was attributed to specimen tortuosity from biopsy and pathologist inexperience with this rare condition.

Implications:

  • This case highlights the importance of considering hyperplasia in the differential diagnosis of biliary tract polypoid lesions.
