Summary

Oral inflammation from dentures may not be due to acrylate allergy. Treatment with antifungal and antiseptic solutions significantly improved symptoms, suggesting other causes for denture stomatitis.

Background:

  • A 70-year-old woman experienced persistent oral mucosal inflammation localized to her upper partial denture area.
  • Previous metal denture was well-tolerated; a new acrylate denture coincided with symptom onset.

Observation:

  • Erythema and edema were sharply demarcated on the gum ridge contacting the denture.
  • Patch testing revealed allergies to multiple acrylates, including methyl methacrylate.
  • Yeast culture was negative for Candida, ruling out fungal overgrowth as the primary cause.

Findings:

  • The patient showed 90% improvement with topical nystatin and chlorhexidine soak, despite dentures not being boiled to polymerize residual monomers.
  • This clinical improvement suggested the acrylate allergy was not the primary driver of the inflammation.
