Summary

Preoperative cholangiography is not essential during laparoscopic cholecystectomy if endoscopic retrograde cholangiopancreatography (ERCP) is available. ERCP effectively diagnoses and treats common bile duct stones, minimizing the need for operative cholangiography.

  • Preoperative ERCP was performed in 107 patients based on indications like abnormal LFTs, jaundice, common bile duct (CBD) stones on US, CBD dilatation, or pancreatitis.
  • Main Results:

    • Preoperative ERCP was performed in 9.6% of patients, with indications including abnormal LFTs (74), jaundice (37), CBD stones (36), CBD dilatation (46), and pancreatitis (20).
    • Common bile duct stones were found and cleared in 38% (41/107) of patients undergoing ERCP.
    • Postoperative ERCP was needed in 8 patients for retained stones or bile leaks. Only two patients had retained CBD stones post-surgery, both successfully treated with ERCP.

    Conclusions:

    • Preoperative cholangiography is not essential for laparoscopic cholecystectomy when ERCP facilities are available.
    • ERCP serves as an effective diagnostic and therapeutic tool for managing biliary tract issues, including common bile duct stones.
    • The study supports a strategy prioritizing ERCP over routine operative cholangiography in select patients.
