Corticosteroids for treating nerve damage in leprosy

Corticosteroids do not significantly improve nerve function in leprosy patients long-term. More research is needed to establish the effectiveness of corticosteroids for leprosy nerve damage and explore new therapies.

Purpose of the Study:
- To evaluate the long-term effects of corticosteroids on nerve damage in individuals with leprosy.

- Systematic review of randomized and quasi-randomized controlled trials identified through extensive database searches (Cochrane, MEDLINE, EMBASE, CINAHL, LILACS) up to January 2006.
- Primary outcome: improvement in sensory and motor nerve function at one year.
- Secondary outcomes included nerve function at two years, pain, tenderness, and adverse events; data were independently extracted and trial quality assessed.
Main Results:
- Three randomized controlled trials involving 513 participants were included.
- Two trials comparing prednisolone with placebo showed no significant difference in nerve function improvement at twelve months.
- A third trial indicated that a 3-month prednisolone course required more additional corticosteroids compared to 5-month regimens for severe reactions; serious adverse events were not significantly more frequent with corticosteroids.
Conclusions:
- Current evidence from randomized controlled trials does not support a significant long-term benefit of corticosteroids for acute nerve damage in leprosy.
- Further high-quality randomized controlled trials are necessary to determine the effectiveness of corticosteroids, optimal treatment regimens, and to investigate novel therapeutic approaches for leprosy-induced nerve damage.
