Summary

Malignant appendiceal neoplasms are rare and difficult to diagnose pre-operatively. Right hemicolectomy is recommended for most, except small, low-risk carcinoid tumors.

Purpose of the Study:

  • To present a 10-year experience with appendiceal neoplasms.
  • To review current epidemiology, pathology, and treatment modalities for appendiceal neoplasms.
  • To identify optimal treatment strategies for malignant appendiceal neoplasms.

Main Methods:

  • Retrospective histopathological review of appendicectomy specimens (April 1994 - December 2003).
  • Analysis of patient demographics, operative details, histopathology, and clinical outcomes.
  • Literature search of PubMed using keywords: appendix, tumour, neoplasm, malignancy.

Main Results:

  • Twenty-two malignant appendiceal neoplasms identified: 11 carcinoid-type, 8 adenocarcinomas, 3 lymphomas.
  • Mean follow-up was 41 months; 14 patients survived.
  • Classical carcinoid tumors (CCT) showed better outcomes than goblet cell carcinoid, adenocarcinoma, and lymphoma.

Conclusions:

  • Right hemicolectomy is the recommended treatment for most malignant appendiceal neoplasms.
  • Exceptions include small CCT (<2 cm, low proliferative index, no extension).
  • Adjuvant therapy should be considered post-oncological assessment.
