Summary

A rare sterile abscess at the pyramidal apex, mimicking a tumor, was found in a patient with cranial nerve deficits. This case highlights the importance of considering abscesses in the differential diagnosis of apical skull base lesions.

Area of Science:

  • Neurology
  • Otolaryngology
  • Radiology

Background:

  • Pyramidal apex abscesses are uncommon but serious conditions.
  • They can present with neurological deficits mimicking tumors.

Observation:

  • A 52-year-old male presented with right-sided cranial nerve V and VII palsies and hearing loss.
  • Imaging suggested a tumor at the right pyramidal apex.
  • Surgery revealed a sterile encapsulated abscess.

Findings:

  • The abscess contained pus but no bacterial agent was isolated.
  • It was adjacent to the sphenoid sinus mucosa and mastoid air cells.
  • Sterile abscesses account for approximately 20% of such cases.

Implications:

  • This case underscores the need to include sterile abscesses in the differential diagnosis of apical skull base masses.
  • Accurate diagnosis is crucial for appropriate management and to avoid unnecessary oncological treatment.
  • Further investigation into the etiology of sterile abscesses is warranted.
