Background:

Thrombocytopenia, a condition characterized by low platelet counts, is often initially suspected as immune thrombocytopenic purpura (ITP).
Accurate diagnosis is essential for appropriate treatment and patient outcomes, especially in resource-limited settings.

Observation:

Two pediatric cases with suspected ITP are presented.
Neither child exhibited physical or peripheral blood smear findings indicative of leukemia at presentation.

Findings:

One child, initially treated for ITP with corticosteroids, was subsequently diagnosed with acute lymphoblastic leukemia.
The second child was diagnosed with acute myeloid leukemia upon bone marrow aspirate examination.

Implications:

Bone marrow examination is a critical diagnostic tool for thrombocytopenia in developing countries.
This evaluation is necessary before concluding a diagnosis of immune thrombocytopenic purpura to avoid misdiagnosis of pediatric leukemia.
Early and accurate diagnosis through bone marrow assessment can significantly alter treatment and improve prognoses for children with hematological malignancies.
