Background:

  • Thrombocytopenia, a condition characterized by low platelet counts, is often initially suspected as immune thrombocytopenic purpura (ITP).
  • Accurate diagnosis is essential for appropriate treatment and patient outcomes, especially in resource-limited settings.

Observation:

  • Two pediatric cases with suspected ITP are presented.
  • Neither child exhibited physical or peripheral blood smear findings indicative of leukemia at presentation.

Findings:

  • One child, initially treated for ITP with corticosteroids, was subsequently diagnosed with acute lymphoblastic leukemia.
  • The second child was diagnosed with acute myeloid leukemia upon bone marrow aspirate examination.

Implications:

  • Bone marrow examination is a critical diagnostic tool for thrombocytopenia in developing countries.
  • This evaluation is necessary before concluding a diagnosis of immune thrombocytopenic purpura to avoid misdiagnosis of pediatric leukemia.
  • Early and accurate diagnosis through bone marrow assessment can significantly alter treatment and improve prognoses for children with hematological malignancies.
