Factitious disease affecting the maxillofacial region is rare. This case highlights self-inflicted oral and facial lesions misdiagnosed as pemphigus vulgaris, emphasizing the need for considering factitious disorders in differential diagnoses.

Background:

  • Factitious disease, characterized by intentional self-harm, rarely affects the maxillofacial region.
  • Reported manifestations include nasal ulceration, facial emphysema, and oral lesions.
  • Differential diagnosis of oral vesiculobullous disorders is complex.

Observation:

  • A young woman presented with unilateral bullous, ulcerative oral, and erythematous facial lesions.
  • Initial diagnosis was pemphigus vulgaris.
  • Lesions were later determined to be self-inflicted.

Findings:

  • This clinical presentation of factitious disease in the maxillofacial region is novel.
  • The case demonstrates a unique manifestation of self-inflicted injuries.
  • The patient's lesions mimicked autoimmune blistering diseases.

Implications:

  • This case expands the known spectrum of factitious disease presentations.
  • Clinicians should consider factitious disorder in the differential diagnosis of unexplained oral and facial lesions.
  • Early recognition of self-inflicted injuries is crucial for appropriate management and treatment.
