The bilateral axillo-breast (BAB) approach for endoscopic total thyroidectomy is a safe and effective surgical option. This method offers excellent cosmetic outcomes with comparable completeness to open thyroidectomy.

Area of Science:

  • Endocrinology
  • Surgical Oncology
  • Minimally Invasive Surgery

Background:

  • Endoscopic techniques are increasingly utilized in thyroid surgery.
  • The bilateral axillo-breast (BAB) approach was developed for total thyroidectomy.
  • This study evaluates the efficacy and safety of the BAB approach.

Purpose of the Study:

  • To assess the completeness of total thyroidectomy using the BAB endoscopic approach.
  • To compare complications between endoscopic and conventional open thyroidectomy.
  • To evaluate cosmetic outcomes of the BAB approach.

Main Methods:

  • Retrospective analysis of 198 open thyroidectomies and 103 endoscopic thyroidectomies for papillary thyroid microcarcinoma.
  • Postoperative thyroglobulin (TG) levels were used to assess completeness.

  • Complications including hypocalcemia and vocal cord palsy were evaluated.
  • Main Results:

    • No significant differences in postoperative TG levels between open and endoscopic thyroidectomy.
    • Transient vocal cord palsy was higher in the endoscopic group but resolved within 3 months.
    • Excellent cosmetic results were observed with the BAB endoscopic approach.

    Conclusions:

    • The BAB endoscopic approach is a feasible method for total thyroidectomy.
    • It demonstrates comparable completeness to open surgery with good cosmetic results.
    • The BAB approach is a suitable surgical option for selected thyroid cancer patients.
