  • Rigid gas permeable (RGP) contact lenses are commonly used to correct vision in keratoconus.
  • Uncorrected residual wavefront error can impact visual performance despite contact lens correction.

Purpose of the Study:

  • To investigate the relationship between uncorrected residual wavefront error and visual performance in keratoconic eyes wearing RGP contact lenses.
  • To determine if higher-order aberrations correlate with visual acuity and contrast sensitivity in this population.

Main Methods:

  • Seven keratoconic eyes wearing RGP lenses were studied.
  • Visual performance was assessed using high-contrast logMAR visual acuity and Pelli-Robson contrast sensitivity.
  • Wavefront error was measured using a Shack-Hartmann sensor and analyzed for higher-order aberrations.
  • Image quality metrics (RMSw, RMSs, Bave, D50) were derived and regressed against visual performance measures.

Main Results:

  • Five of seven eyes showed reduced visual acuity compared to normal values.
  • Two of seven eyes had reduced contrast sensitivity.
  • Five of seven eyes exhibited elevated higher-order root mean square (RMS) wavefront error.
  • Significant correlations were found between visual performance metrics and image quality metrics.

Conclusions:

  • Visual performance is diminished in RGP-wearing keratoconic eyes, associated with increased higher-order wavefront aberrations.
  • Retinal image quality metrics highlight the link between keratoconic visual performance and residual wavefront errors.
  • Targeting the correction of these aberrations may lead to improved visual outcomes for keratoconus patients.
