Establishment and Evaluation of a Risk Prediction Model for Pathological Escalation of Gastric Low-Grade Intraepithelial Neoplasia

Purpose of the Study:

  • To identify independent prognostic factors for large gastric cancer using multivariate analysis.
  • To clarify the impact of various clinicopathologic factors on survival.

Main Methods:

  • Analysis of 95 patients who underwent gastrectomy for large gastric cancer.
  • Evaluation of 24 clinicopathologic factors (patient, operation, tumor).
  • Survival analysis using Kaplan-Meier, Mantel-Cox, and Cox proportional hazards models.

Main Results:

  • Overall 5-year survival was 22%, with a median survival of 15 months.
  • Significant prognostic factors included tumor size, gross type, serosal invasion, lymph node metastasis, liver metastasis, peritoneal dissemination, disease stage, resection margin, and operative curability.
  • Independent prognostic factors identified were serosal invasion, extragastric lymph node metastasis, and liver metastasis.

Conclusions:

  • Serosal invasion, extragastric lymph node metastasis, and liver metastasis are independent prognostic factors in large gastric cancer.
  • Prognosis is primarily determined by these tumor characteristics, independent of patient or surgical factors.
