Background:

  • Carotid-cavernous fistula (CCF) is an infrequent complication of severe craniocerebral trauma.
  • Prompt diagnosis and treatment of CCF are essential to prevent serious complications, including vision loss.

Observation:

  • A 27-year-old male sustained severe craniocerebral injury from a motor vehicle accident, necessitating emergent craniotomy.
  • Three months post-injury, the patient developed progressive chemosis and proptosis of the left eye.

Findings:

  • Computed tomography and cerebral angiography confirmed a carotid-cavernous fistula.
  • Angiography identified a direct communication between the carotid artery and the cavernous sinus.
  • Transarterial embolization achieved immediate and permanent occlusion of the fistula.
