A rare case of fungal keratitis caused by Aspergillus flavus led to malignant glaucoma. Topical steroids may have worsened the fungal infection, necessitating advanced surgical intervention beyond standard keratoplasty.

Area of Science:

  • Ophthalmology
  • Medical Mycology

Background:

  • Fungal keratitis is a serious ocular infection.
  • Malignant glaucoma is a rare but severe complication of ocular conditions.

Observation:

  • A 48-year-old woman presented with a severe corneal ulcer after 10 days of topical steroid use.
  • Microbiological analysis identified Aspergillus flavus as the causative agent.
  • The patient developed nonresponsive high intraocular pressure and a shallow anterior chamber.

Findings:

  • The patient was diagnosed with keratomycotic malignant glaucoma.
  • Initial treatment with therapeutic keratoplasty and cataract extraction was followed by recurrence.
  • A subsequent limited pars plana vitrectomy was performed.

Related Experiment Videos

Implications:

  • Aspergillus-induced keratomycotic glaucoma is a previously unreported complication.
  • Topical steroid use may predispose to fungal penetration and glaucoma formation.
  • Management may require surgical intervention beyond keratoplasty, including vitrectomy.
