Summary

Cerebrovascular vasomotor reactivity in multiple sclerosis (MS) patients was assessed using breath holding (BH) and transcranial Doppler (TCD). Results showed no significant differences in reactivity between MS patients and healthy controls across different disease stages, suggesting normal cerebrovascular function.

Background:

  • Multiple sclerosis (MS) exhibits a known association with cerebral vasculature.
  • Vascular endothelial cell activation and ischemic basis for demyelination are hypothesized early MS events.
  • Breath holding (BH) induces hypoxia, leading to cerebral blood flow (CBF) changes measurable by transcranial Doppler (TCD).

Purpose of the Study:

  • To investigate cerebrovascular integrity and vasomotor reactivity in relapsing-remitting MS (RRMS) patients.
  • To assess reactivity during acute exacerbation, post-treatment, and remission phases.
  • To compare MS patient reactivity with healthy controls using TCD.

Main Methods:

  • 12 RRMS patients and 11 healthy controls underwent TCD examinations.
  • Measurements included cerebral blood flow velocities during normal breathing and 15-second BH.
  • Vasomotor reactivity was calculated as the percentage change in CBF velocities during BH.

Main Results:

  • No significant differences in vasomotor reactivity were found between RRMS patients and controls (55.7% vs. 46.5% during attacks, 48.3% post-treatment, 50.9% in remission).
  • Vasomotor reactivity remained consistent among MS patients throughout the study's different disease activity stages.
