Summary

Lower tacrolimus doses are crucial for Dutch-Belted rabbits to prevent toxicity. A modified regimen of 0.08 mg/kg/d, with blood level monitoring, improved survival rates in experimental transplants.

Area of Science:

  • Immunology
  • Pharmacology
  • Veterinary Medicine

Background:

  • Tacrolimus is a vital immunosuppressant used in clinical practice.
  • Effective immunosuppression relies on therapeutic drug monitoring to avoid toxicity.
  • Previous studies often lacked blood level determinations, leading to adverse effects.

Purpose of the Study:

  • To investigate the toxic effects of intramuscular tacrolimus in Dutch-Belted rabbits.
  • To determine modified dosage regimens for improved animal survival.
  • To establish safe tacrolimus blood concentrations in experimental models.

Main Methods:

  • Three tacrolimus dosage regimens (1 mg/kg/d, 0.2 mg/kg/d, 0.08 mg/kg/d) were tested in Dutch-Belted rabbits.
  • Drug concentrations were guided by human therapeutic values (5-20 ng/mL).

  • Euthanasia criteria included significant weight loss, cardiopulmonary failure, or elevated creatinine levels.
  • Main Results:

    • The initial 1 mg/kg/d dosage resulted in a mean blood level of 90.7 ng/mL, leading to death in 10/12 rabbits.
    • A reduced dosage of 0.2 mg/kg/d yielded a mean blood level of 17.6 ng/mL, but 8/10 rabbits still died.
    • The 0.08 mg/kg regimen achieved a mean blood level of 6.8 ng/mL, with 18/20 rabbits surviving.

    Conclusions:

    • Dutch-Belted rabbits necessitate lower intramuscular tacrolimus dosages than other breeds for safe immunosuppression.
    • A regimen of 0.08 mg/kg/d, coupled with drug level monitoring, is effective for survival.
    • This study highlights breed-specific dosage requirements for tacrolimus in rabbits.
