Background:

Purpose of the Study:

  • To investigate provider-level factors associated with inappropriate PSA screening in VHA hospitals.
  • To identify determinants influencing deviations from evidence-based PSA screening guidelines.

Main Methods:

  • Analysis of PSA test usage data from VHA databases (1997-2004) for 181,139 male patients and 4,823 healthcare providers.
  • Exclusion of tests for non-screening reasons and definition of inappropriate use as screening in men outside the 40-75 age range.
  • Application of univariate and multivariate Poisson regression models to identify significant determinants.

Main Results:

  • Approximately 19.3% of PSA tests were deemed inappropriate, primarily in men over 75 (18.4%).
  • Urology specialists, male providers, infrequent PSA testers, and affiliation with certain hospitals showed higher inappropriate screening rates.
  • Infrequent testing and hospital affiliation remained significant predictors in multivariate analysis; older male providers showed increased inappropriate screening.

Conclusions:

  • Provider-level factors significantly influence the misuse of PSA screening, indicating frequent disregard for evidence-based guidelines.
  • Further research is needed to understand the role of potential biases, such as 'prostatempathy,' in PSA screening misuse by older male providers.
