Area of Science:

  • Dermatology
  • Immunodermatology
  • Autoimmune Blistering Diseases

Background:

  • Dexamethasone-cyclophosphamide pulse (DCP) therapy is a common treatment for pemphigus in India.
  • Cyclophosphamide in DCP therapy is associated with significant adverse effects.
  • Limitations of standard DCP therapy necessitate exploring alternative treatment protocols.

Purpose of the Study:

  • To evaluate the efficacy and utility of modified pulse therapy for pemphigus.
  • To assess treatment outcomes with modifications to the original DCP protocol.
  • To explore the potential of azathioprine or methotrexate as cyclophosphamide alternatives.

Main Methods:

  • A cohort of 41 pemphigus patients received modified pulse therapy between 1995 and 2001.
  • Modifications included protocol adjustments and cyclophosphamide substitution with azathioprine or methotrexate in select cases.

  • Treatment adherence and response were monitored.
  • Main Results:

    • A good therapeutic response was observed in 34 patients who consistently followed the treatment regimen.
    • The modified pulse therapy demonstrated clinical effectiveness in managing pemphigus.
    • No specific mention of side effect profile in the results section.

    Conclusions:

    • Modifications to the standard Dexamethasone-cyclophosphamide pulse (DCP) therapy protocol are highly effective.
    • The adapted pulse therapy approach offers a beneficial and useful treatment strategy for pemphigus patients.
    • This study supports the clinical utility of modified pulse therapy in pemphigus management.
