Summary

Patients with pulmonary hypertension (PH) have a higher prevalence of thyroid disease. Early thyroid function evaluation in PH patients is recommended to identify and manage coexisting thyroid conditions.

Background:

  • The relationship between thyroid dysfunction and pulmonary hypertension (PH) remains incompletely understood.
  • Previous studies have not thoroughly investigated the association between these two conditions.

Purpose of the Study:

  • To determine the prevalence of thyroid disease in patients diagnosed with pulmonary hypertension.
  • To compare thyroid disease rates in PH patients versus a control group without PH.

Main Methods:

  • A retrospective analysis of 356 PH patients and 698 sex-matched controls was conducted.
  • Thyroid disease was identified by abnormal thyroid-stimulating hormone or thyroxine levels, thyroid disease history, or elevated thyroperoxidase antibodies.
  • The primary outcome was the prevalence of thyroid disease in both groups.

Main Results:

  • The prevalence of thyroid disease was significantly higher in PH patients (24%) compared to controls (15%).
  • This association was particularly strong in World Health Organization group 1 pulmonary artery hypertension (OR, 2.53; P < 0.001).
  • Most affected PH patients had mild hypothyroidism, with 16% of cases being previously undiagnosed.

Conclusions:

  • Pulmonary hypertension patients exhibit a greater prevalence of thyroid disease.
  • Screening for thyroid dysfunction in individuals with PH is advisable for timely diagnosis and management.
