Double common bile duct: a case report.

Background:

• Double common bile duct (DCBD) is a rare congenital anomaly with significant clinical implications.
• DCBD is often associated with biliary lithiasis, choledochal cysts, anomalous pancreaticobiliary junction (APBJ), and gastrointestinal malignancies.

Observation:

• A rare case of DCBD was identified, presenting with APBJ and gallstones within the more developed common bile duct.
• The accessory common bile duct (ACBD) in this case drained into the pancreatic duct.

Findings:

• The anomaly was initially suspected via transabdominal ultrasonography.
• Endoscopic retrograde cholangiopancreatography (ERCP) confirmed the diagnosis and facilitated endoscopic sphincterotomy and stone extraction.

Implications:

• DCBD with ACBD opening into the pancreatic duct is frequently linked to APBJ and gallbladder carcinoma.
• The specific opening site of the ACBD is crucial for understanding associated pathologies.
• Timely and accurate diagnosis of DCBD is vital to prevent potential operative complications.
