Area of Science:

  • Medical Microbiology
  • Infectious Diseases
  • Dermatology

Background:

  • Actinomycetoma treatment often requires prolonged, supervised parenteral antibiotics.
  • A novel treatment schedule combining short-term parenteral therapy with extended oral medication is proposed.

Purpose of the Study:

  • To evaluate the efficacy and convenience of a new treatment regimen for actinomycetoma.
  • To assess a treatment protocol involving an initial intensive parenteral phase followed by a maintenance oral phase.

Main Methods:

  • Sixteen patients with presumptive actinomycetoma received intravenous gentamicin and oral cotrimoxazole for four weeks.
  • A maintenance phase with oral cotrimoxazole and doxycycline was administered until sinus healing.
  • Treatment duration averaged 5-6 months, with follow-up assessments.

Published on: August 15, 2025

Main Results:

  • All 16 patients showed significant improvement after the intensive phase.
  • Nine patients achieved complete sinus healing with reduced swelling and induration.
  • Six patients remained disease-free post-treatment; two experienced side effects requiring cotrimoxazole withdrawal.

Conclusions:

  • The described regimen is effective for treating actinomycetoma.
  • The reduced duration of parenteral therapy enhances treatment convenience.
