Background:

  • Splenic tuberculosis is an uncommon diagnosis in clinical settings.
  • Tuberculosis (TB) can manifest in unusual organs, particularly in immunocompromised individuals.

Observation:

  • Three patients with acquired immunodeficiency syndrome (AIDS) presented with fever and constitutional symptoms.
  • Abdominal sonography showed multiple hypoechoic splenic lesions.
  • Abdominal computed tomography (CT) revealed multiple hypodense splenic lesions.

Findings:

  • Needle aspiration of the spleen under sonographic guidance diagnosed splenic tuberculosis in two patients.
  • Splenectomy confirmed the diagnosis in the third patient.
  • Imaging findings (hypoechoic/hypodense lesions) are key indicators.
