Summary

Common variable immunodeficiency (CVID) can present with significant digestive issues like chronic diarrhea and malabsorption. Immunoglobulin replacement therapy offers a favorable prognosis for these CVID patients.

Area of Science:

  • Immunology
  • Gastroenterology
  • Clinical Medicine

Background:

  • Common variable immunodeficiency (CVID) is a primary immunodeficiency characterized by low immunoglobulin levels and impaired B-cell differentiation.
  • Digestive manifestations are increasingly recognized in CVID, impacting patient morbidity.
  • Understanding these gastrointestinal (GI) symptoms is crucial for timely diagnosis and management.

Observation:

  • Four CVID cases with predominant GI symptoms were analyzed.
  • Chronic diarrhea and intestinal malabsorption were consistent findings across all cases.
  • Associated conditions included lymphoid nodular hyperplasia, Giardia infestation, chronic pancreatitis, aphthous stomatitis, chronic hepatitis, and sideroblastic anemia.

Findings:

  • All patients received immunoglobulin (Ig) replacement therapy, showing favorable clinical evolution over at least 14 months.

Related Experiment Videos

  • One patient exhibited an elevated suppressor T lymphocyte count with an inverted T helper/T suppressor ratio, which improved with cimetidine.
  • The study highlights the diverse and severe GI manifestations associated with CVID.
  • Implications:

    • Early recognition of GI symptoms in CVID is vital for effective management.
    • Immunoglobulin therapy is a cornerstone treatment, leading to positive outcomes.
    • Further research into immune dysregulation in CVID-associated GI conditions may reveal novel therapeutic targets.
