Bronchography in patients with hemoptysis.

J V Forrest, S S Sagel, G H Omell

    AJR. American Journal of Roentgenology

    Summary

Bronchography offers limited value for evaluating hemoptysis (coughing up blood). Advanced diagnostic tools like fiberoptic bronchoscopy have largely superseded its use, even with abnormal chest X-rays.

    Area of Science:

    • Pulmonology
    • Diagnostic Imaging

    Background:

    • Hemoptysis evaluation often involves imaging and direct visualization.
    • Bronchography was historically used but its utility is debated.

    Purpose of the Study:

    • To assess the diagnostic value of bronchography in hemoptysis evaluation.
    • To determine if bronchography is necessary when other diagnostic methods are normal or abnormal.

    Main Methods:

    • Retrospective analysis of 196 patient cases.
    • Review of findings from bronchography, chest roentgenography, and bronchoscopy.

    Main Results:

    • Bronchography demonstrated limited value in the overall evaluation of hemoptysis.
    • Patients with a single hemoptysis episode and normal initial findings did not benefit from bronchography.
    • Even with abnormal chest roentgenograms, advanced procedures replaced bronchography's utility.

    Conclusions:

    • Bronchography is largely unnecessary for hemoptysis evaluation.
    • Modern cytologic and biopsy techniques are superior to bronchography.
    • The diagnostic yield of bronchography is minimal in the current medical landscape.
