Background:

The Allen test is a standard pre-operative assessment for hand circulation.
Radial artery harvesting is common in coronary artery bypass grafting (CABG).
Ensuring adequate hand perfusion is critical before radial artery removal.

Purpose of the Study:

To evaluate the sufficiency of the Allen test for guiding radial artery harvesting in CABG patients.
To compare the diagnostic accuracy of the Allen test against objective vascular assessments.

Main Methods:

145 patients scheduled for CABG underwent the Allen test, Doppler ultrasonography, and digital plethysmography.
Ultrasonography included anatomical and circulatory assessments.
Sensitivity, specificity, and diagnostic accuracy of the Allen test were calculated.

Main Results:

The Allen test showed 73.2% sensitivity and 97.1% specificity.
23% of patients had a positive (abnormal) Allen test.
Ultrasound identified anatomical and/or circulatory abnormalities in a subset of patients.

Conclusions:

The Allen test is a valid screening tool for hand circulation.
A negative Allen test suggests safe radial artery harvesting.
Positive Allen test results necessitate further vascular evaluation prior to harvesting.
