  • A 61-year-old woman presented with shock, dyspnea, and lower extremity swelling three weeks post-mitral valve replacement.
  • Clinical examination revealed signs of heart failure and a distinct murmur.
  • Transesophageal echocardiography identified an unusual echo-lucent area in the left atrium, suggesting a separate chamber.

Findings:

  • The patient had a large, separate chamber in the lateral left atrium communicating with the left ventricle.
  • Intraoperative findings confirmed left atrial dissection.
  • The patient could not be weaned from cardiopulmonary bypass and died during surgery.

Implications:

  • This case underscores the potential for rare, severe complications following mitral valve surgery.
  • Left atrial dissection presents a significant surgical challenge with high mortality.
  • Further research into identifying and mitigating risks associated with rheumatic heart disease surgery is warranted.
