Background:

Laparoscopic cholecystectomy is a widely performed surgical procedure.
Serious complications can arise, necessitating prompt recognition and intervention.

Purpose of the Study:

To analyze the incidence and types of major complications during laparoscopic cholecystectomy.
To emphasize the importance of immediate management for safe outcomes.

Main Methods:

Retrospective review of 1225 laparoscopic cholecystectomies performed over a 12-year period.
Detailed documentation of all major complications, including type, incidence, and management strategies.

Main Results:

A total of 19 major complications (1.5%) occurred, with a 7.4% conversion rate.
Specific complications included common bile duct injury (0.16%), vessel injury (0.32%), bleeding (0.8%), bile leak (0.08%), and duodenum injury (0.08%).
Complications were recognized intraoperatively in 11 cases, with reoperation in 5 cases (0.4%).

Conclusions:

Major complications, though infrequent, can occur during laparoscopic cholecystectomy.
Timely identification and appropriate management are critical for ensuring patient safety and favorable results.
