Deep vein thrombosis--a rare post transplant complication

Deep vein thrombosis (DVT) is a rare complication after stem cell transplant (SCT). Careful monitoring of anticoagulation therapy is crucial to prevent fatal bleeding events in high-risk patients.

- Deep vein thrombosis (DVT) is a rare but serious condition that can occur after organ or stem cell transplantation.
- It often results from a combination of risk factors, including venous stasis, making diagnosis and management critical.
Observation:
- A 7-year-old boy with B-thalassaemia major developed DVT four months post-SCT.
- He was treated with anticoagulants, including low molecular weight heparin and warfarin sodium, with INR stabilized between 2.5-3.0.
Findings:
- Despite therapeutic anticoagulation, the patient later presented with bleeding diathesis.
- The patient died due to intracranial hemorrhage, attributed to excessive and unchecked anticoagulation.
Implications:
- This case highlights the critical need for strict monitoring of anticoagulation therapy in post-transplant patients.
- Close monitoring of PT/INR is essential to prevent potentially fatal bleeding complications from over-anticoagulation.
