Summary

Broncholithiasis, a lung condition, can be challenging to manage. This case report highlights cryotherapy via flexible bronchoscopy as a safe and effective method for removing broncholiths, resolving obstruction and recurrent pneumonia.

Background:

  • Pulmonary broncholithiasis presents management challenges, often necessitating invasive procedures like rigid bronchoscopy or surgery.
  • Broncholiths can cause obstructive symptoms and recurrent pneumonia, impacting patient quality of life.

Observation:

  • A 38-year-old male with chronic cough, dyspnea, and recurrent pneumonia exhibited hilar, mediastinal, and right middle lobe bronchial calcifications.
  • Initial attempts to remove the obstructing broncholith using forceps during flexible bronchoscopy were unsuccessful.

Findings:

  • Cryotherapy via flexible bronchoscopy successfully removed the obstructing broncholith with minimal bleeding.
  • Histopathological examination confirmed the broncholith and identified Histoplasma fungal forms.

Implications:

  • Cryotherapy offers a safe and effective alternative for removing partially attached broncholiths using flexible bronchoscopy.
  • This approach can resolve bronchial obstruction and associated symptoms like recurrent pneumonia, avoiding more invasive interventions.
