    • Perivascular Epitheloid Cell tumors (PEComas) are rare mesenchymal neoplasms with uncertain differentiation.
    • PEComas can occur in various locations, but pericardial involvement is exceptionally uncommon.
    • This case highlights a unique presentation of PEComa in the pericardial space.

    Observation:

    • A 19-year-old male presented with dyspnea and chest pain.
    • Echocardiography revealed a large, posterior pericardial mass compressing the left atrium, with moderate pericardial effusion.
    • Histopathology confirmed neoplastic cells with HMB-45 positivity, characteristic of PEComa.

    Findings:

    • The study describes the first documented case of a giant pericardial PEComa.
    • The tumor was successfully diagnosed through histopathological analysis and immunohistochemistry.
    • The patient experienced an uneventful six-month follow-up post-diagnosis.

    Implications:

    • This case expands the known spectrum of PEComa locations.
    • It underscores the importance of considering rare tumors in the differential diagnosis of cardiac masses.
