Methods of Documentation VII: EMR

Electronic Medical Records (EMRs) primarily center around electronically documenting patients' health information within a single healthcare organization or practice. They contain essential clinical data related to a patient's medical history, diagnoses, medications, treatment plans, lab results, and other pertinent information relevant to the specific encounter or episode of care. EMRs are designed to streamline documentation and workflow processes within individual healthcare settings,...

Cardiopulmonary Resuscitation II: ACLS Airway Management

Airway management is a key skill in emergency and critical care settings, as maintaining a clear airway is essential for adequate oxygenation and ventilation.Head Tilt-Chin Lift TechniqueThe head tilt-chin lift maneuver is an essential technique primarily used in patients without suspected cervical spine injuries. To perform this maneuver, one hand is placed on the patient’s forehead, and gentle pressure is applied backward to tilt the head. The fingertips of the other hand are positioned under...

Summary

Reserving operating room (OR) capacity for emergency surgery in all elective ORs significantly reduces emergency patient waiting times. This approach also decreases staff overtime and improves overall OR utilization, proving more efficient than dedicated emergency ORs.

Area of Science:

  • Health Services Research
  • Operations Research
  • Surgical Logistics

Background:

  • Long waiting times for emergency operations are linked to increased postoperative complications and patient morbidity.
  • Reserving Operating Room (OR) capacity is a strategy to enhance OR responsiveness for emergency cases.

Purpose of the Study:

  • To determine the optimal strategy for reserving OR time to improve emergency surgery outcomes.
  • To compare two distinct OR capacity reservation approaches: dedicated emergency ORs versus distributed capacity in elective ORs.

Main Methods:

  • A discrete event simulation model was employed to replicate real-world OR operations.
  • Key performance indicators evaluated included emergency surgery waiting times, staff overtime, and OR utilization rates.

Main Results:

  • Reserving capacity across all elective ORs reduced emergency surgery waiting times from 74 minutes to 8 minutes.
  • Staff overtime decreased by 20%, and overall OR utilization increased by approximately 3% with this approach.
  • The study demonstrated that utilizing elective ORs for emergency cases is more efficient than using a dedicated Emergency OR.

Conclusions:

  • Distributing reserved OR capacity across elective ORs is a superior strategy for managing emergency surgical cases.
  • The findings supported the closure of the dedicated Emergency OR at Erasmus MC, optimizing resource allocation.
