A new acne grading system, the Comprehensive Acne Severity Scale (CASS), effectively assesses facial and truncal acne severity. CASS demonstrates strong validity and is simpler for clinical use than existing scales.

Main Results:

Significant Spearman correlations observed between CASS and Leeds grades for face (0.823), chest (0.854), and back (0.872) (p < .001).
Changes in CASS and Leeds scores after 6 months of treatment were also significantly correlated across all sites (p < .001).
Demonstrated strong concurrent validity and responsiveness of the CASS system.

Conclusions:

The Comprehensive Acne Severity Scale (CASS) exhibits strong concurrent validity when compared to the Leeds grading system.
CASS is a simpler and more practical tool for clinical practice and research.
This validated system offers improved assessment for acne involving the face, chest, and back.
