Area of Science:

  • Cytopathology
  • Surgical Pathology
  • Diagnostic Accuracy

Background:

  • Intraoperative fine needle aspiration (FNA) cytology aids in rapid diagnosis.
  • Conventional cytological preparation methods have limitations in accuracy and adequacy.
  • The ThinPrep method offers potential improvements in sample preparation.

Purpose of the Study:

  • To compare the diagnostic accuracy of the ThinPrep method versus conventional methods for FNA cytology.
  • To evaluate specimen adequacy and turn around time (TAT) for both preparation techniques.

Main Methods:

  • Fifty fresh tumors were aspirated and prepared using four methods: direct smear (H&E, Pap stain) and ThinPrep (H&E, Pap stain).
  • Cytopathologists evaluated slides for preparation quality, diagnostic accuracy, and interpretation time.

  • Accuracy was defined as absolute agreement with histopathological diagnosis.
  • Main Results:

    • The ThinPrep method with Pap stain demonstrated the highest accuracy and specimen adequacy.
    • Histological analysis revealed 43 malignant, 6 benign lesions, and 1 atypical lipoma.
    • Turn around time (TAT) was longer for the ThinPrep method.

    Conclusions:

    • ThinPrep slides with Pap stain provide the best accuracy and specimen adequacy for intraoperative FNA cytology.
    • The increased accuracy of ThinPrep is counterbalanced by a longer TAT.
    • Optimizing TAT for ThinPrep may enhance its clinical utility.
