Background:

Traumatic anterior shoulder dislocation is uncommon in pediatric patients, with treatment data often extrapolated from adult studies.
Limited evidence exists regarding recurrence rates and optimal management in very young children.

Observation:

A 2-year-old toddler sustained a traumatic anterior shoulder dislocation from a pull on an abducted arm.
The injury was treated with closed reduction using the Milch maneuver and short-sling immobilization.

Findings:

A 2.5-year follow-up revealed a stable shoulder with a full range of motion.
A Hill-Sachs lesion was noted despite successful reduction and immobilization.

Implications:

This case suggests that traumatic shoulder dislocations in toddlers may potentially be managed less aggressively post-reduction compared to adults.
Further research is needed to determine the recurrence rate and long-term outcomes in this specific pediatric population.
