Context:

Perendoscopic biliary manometry has limitations due to short recording times and duodenal interference.
Percutaneous transhepatic biliary manometry (PTBM) allows for extended, comfortable monitoring of the sphincter of Oddi (SO).

Purpose:

To investigate human SO motility and its correlation with the small bowel's migrating motor complex (MMC) during fasting.
To explore the potential of PTBM for studying SO motility and related physiological reflexes.

Summary:

Long-term PTBM revealed MMC-like cyclic activities in the SO, termed SO MMC, which correlated with small bowel MMC.
SO was not quiescent during phase I of small bowel MMC, and basal SO pressure varied with SO motility phases.
PTBM enables prolonged studies of SO motility, including physiological reflexes and post-cholecystectomy effects.

Impact:

Provides new insights into the coordinated motility patterns of the human SO and small intestine.
Establishes PTBM as a valuable tool for long-term investigation of SO physiology and related clinical conditions.
Enhances understanding of normal and altered SO function in gastrointestinal motility.
