Area of Science:

  • Pharmacology
  • Toxicology
  • Cardiology

Background:

  • Chloral hydrate is commonly prescribed for insomnia.
  • Accidental overdose can occur from repeated therapeutic dosing.
  • Understanding overdose effects is crucial for patient safety.

Observation:

  • A 42-year-old woman ingested an estimated 8g of chloral hydrate over time.
  • Symptoms included transient loss of consciousness.
  • Severe cardiac arrhythmia, specifically ventricular bigeminy, was observed.

Findings:

  • The patient required intensive care unit admission.
  • Intravenous lignocaine administration was necessary for two days.
  • The cardiac arrhythmia resolved with treatment.

Implications:

  • This case highlights the potential toxicity of chloral hydrate even at doses near therapeutic levels.
  • Monitoring for cardiac events in patients using chloral hydrate is important.
  • Prompt medical intervention is effective in managing chloral hydrate toxicity.
