Summary

This case report details a rare cutaneous chondroblastic osteosarcoma, a malignant soft tissue tumor. Early diagnosis via skin biopsy is crucial, even without visible bone formation, for effective treatment of this extraskeletal neoplasm.

Background:

  • Extraskeletal osteosarcoma (ESOS) is a rare malignant neoplasm typically found in deep soft tissues.
  • Cutaneous involvement is exceptionally uncommon, posing diagnostic challenges.

Observation:

  • A 60-year-old male presented with a rapidly growing cutaneous mass on his right lower thigh.
  • Initial incisional biopsy revealed a dermal and subcutaneous neoplasm with high-grade chondroid features, atypia, and mitotic activity, but no osteoid or bone.

Findings:

  • Histopathology confirmed a high-grade chondroblastic osteosarcoma, characterized by a predominant chondroid matrix with focal fibrosarcomatous areas, giant cells, and eventual bone/osteoid formation.
  • Imaging excluded osseous or other primary tumor origins.
