Generalized subcutaneous edema can be a rare symptom of inflammatory myopathy. This case study highlights dermatomyositis as a cause of severe edema, managed with immunosuppressive therapy.
Area of Science:
- Rheumatology
- Dermatology
- Neurology
Background:
- Inflammatory myopathies, including dermatomyositis, are rare autoimmune diseases.
- Generalized subcutaneous edema is an unusual presentation of dermatomyositis.
Observation:
- A 48-year-old female presented with severe generalized edema, skin rash, dysphagia, and proximal muscle weakness.
- Diagnostic workup included clinical examination, elevated muscle enzymes, electromyography, and muscle biopsy, confirming dermatomyositis.
- MRI showed increased signal intensity in muscle and subcutaneous tissues, ruling out other causes of edema.
Findings:
- The patient's generalized edema was determined to be secondary to dermatomyositis.
- High-dose glucocorticoids and immunosuppressive agents were initiated for treatment.
Implications:
- This case underscores the importance of considering dermatomyositis in patients with unexplained generalized edema.
- Effective management of subcutaneous edema in dermatomyositis may require aggressive immunosuppressive therapy.
